Case Management
Allows for a continuum of care for guests of HONOR’s shelters, and A Friend’s House.
Once guests are settled in the shelter and feel secure in that they have a warm and dry place to sleep and the promise of three meals a day, they are assessed to find out what additional services are necessary. HONOR’s case management staff works with each guest to ensure that any medical or mental health issues are addressed and transportation is provided to and from appointments or school. We also work with each resident to develop a mutually formulated independent living plan. The goal is to help residents gain access to the services and knowledge they need to begin living independently.

Aftercare Case Management
Continued care for those who have been discharged from our shelters to ensure ongoing independence.
HONOR provides continued care to 30-50 people throughout the Orange County community guests who have been positively discharged from one of our shelters or HONOR Recovery. Our staff helps our former guests to continue involvement with the services they need so that ongoing independence is enjoyed. We are here for whatever they need, whether it be allowing them to do laundry or providing them with food from the food pantry, we are there to provide hope and help them to preserve their dignity. The goal is that another episode of homelessness does not occur.

Financial Case Management
Provides financial management for clients throughout Orange County to ensure housing stability.
HONOR’s provides over 180 individuals the assistance they need to help them maintain their finances. Our representatives provide payee-ship for those receiving Social Security Insurance. We take care of paying their expenses in order to ensure permanent housing stability with the goal of homelessness prevention.
Improving Health & Wellness Through Care Management
Health Homes Outreach
Seeks to improve the overall health and wellness of people throughout Orange County.
HONOR’s care managers are assigned to those receiving Medicaid and have chronic medical issues or behavioral issues. HONOR’s care managers do follow ups and make sure they are taking necessary medication and doing preventative care.

Health Home Care Management
Care for the whole person with the goal to improve health and wellness.
We provide assistance for those with Medicaid who have two or more chronic medical conditions, a mental health diagnosis, have tested HIV positive, or have received substance abuse diagnosis. HONOR’s care managers assist with coordinating not only medical care but also other needed community services. The goal is to improve your health and wellness by taking regular medications and doing preventative care.

- Transportation To and From Appointments
- Assistance with Referrals for Medical, Mental or Substance Abuse services
- Follow Up with Specialists
- Assistance with Applying for Benefits
- Referrals to Programs in the Community
