Comprehensive Care Management
HONOR works to ensure clients have access to all types of health services and receive the care they need. From medical screenings to transportation to appointments, to helping navigate more complex or chronic health problems, HONOR provides the coordination and management necessary to get well and stay healthy.
Our goal is to provide clients with access to necessary services and knowledge, empowering them towards independent living, while preserving their dignity every step of the way.
Working in collaboration with Hudson Valley Care and New York State programs, our Care Management includes:
- Personalized care plans
- Open communication to address medical and/or mental health concerns
- Connects clients to mental health and substance abuse disorder providers
- Provides access to urgent medical and social services
- Reduces barriers to care by providing transportation to appointments
- Ensures complex healthcare needs are addressed

Financial Case Management
HONOR’s Financial Case Management Services support the day-to-day and essential needs of clients throughout the Hudson Valley. Our dedicated team assists individuals in managing their finances effectively, as well as longer term planning to rebuild their lives toward a more stable financial future.
